I understand that I will be undertaking a diet and exercise program. These involve making healthy changes to lifestyle and training in a gym environment and there are risks associated with this. If you require an induction to a gym then do so prior to participation. If you require advice on the use of any equipment get someone to show you how to do the exercise safely in person. Any exercises prescribed that you are not happy with, please let us know and we can work around them.
Exercise causes positive changes across the whole body in particular to the heart, lungs, muscles, bones and connective tissues (tendons, ligaments etc) as well as on a cellular level. Additionally there are many positive psychological effects of exercise.
The aim is to become fitter, stronger and more flexible, gain muscle, lose fat and generally improve yourself both physically and mentally. However, as with anything there are risks involved. The reaction of the body to diet and exercise is not predictable. These risk include but are not limited to injury, strains and pain and Project Revitalization shall not be liable from any damage arising from injuries sustained while following a program. Any use of equipment or exercises performed is at your own risk and you assume full responsibility for any injury or damages sustained related to this. In addition to these there is a chance of blood pressure irregularities and heart problems including sudden cardiac death and heart attack. If you have any concerns please visit your doctor prior to participating in any program. I hereby fully and forever release and discharge Project Revitalization, their assigns and agents from all claims, demands, damages, rights of action, present and future therein. I understand and warrant, release and agree that I am in good physical condition and that I have no disability, impairment or ailment preventing me from engaging in active or passive exercise that will be detrimental to heart, safety, or comfort, or physical condition if I engage or
participate. I state that I have had a recent physical check-up and have my personal physician's permission to engage in aerobic and/or anaerobic conditioning.
Project Revitalization (Oliver Matthews and Emil Hodzovic) am committed to using my skills and knowledge to design a safe diet and training program allowing you to achieve any goals specified. I will provide coaching, supervision, advice and support as required and as laid out in the chosen program description.
Client / Trainer Agreement
Progress will be regularly monitored and programs and diet will be revised and adjusted accordingly. It is for you, as the client to let us know if there are any problems or any reasons you are not happy or not making progress. In return, we ask for you to be honest in your reports to us and you do need to tell us if you are not managing to train or are eating extra or different foods beyond those specified in the training program. Failure for you, the client, to check in on a weekly basis and to not adhere to the diet and training plan to the satisfaction of us, the trainers will result in lack of results. There are no refunds available and if the program is stuck to, actively participated in and carried out with enthusiasm there will be no need for them. All client information will be kept strictly private and confidential. If the trainers requires further medical information from a practitioner, the client must provide such details. It is understood between client and trainer that both will commit to the programme and give 100% effort.
